What is Wraparound?
Wraparound is a comprehensive, strengths-based, and needs-driven support system approach to care for families with youth 0-21, who have serious emotional or behavioral challenges and are at risk of out-of-home placement. Our Wraparound team works with families in their homes and communities. Our trained Wraparound team helps families to identify and achieve positive goals through building on strengths, learning how to access community resources and bolstering natural supports.

Key Principles of Wraparound Services

Team-Based Approach
Wraparound services prioritize common goals identified by the child or youth, their family, and care team leadership. This collaborative effort involves trusted friends, providers, and community members who work together to create and implement customized care plans.

Methodical Support
Wraparound programs encompass four key phases, including engagement and team preparation, initial plan development, implementation, and transitioning out of formalized wraparound support. These phases aim to stabilize crisis situations, address safety concerns, and celebrate successes while addressing concerns proactively.

Holistic Care
Wraparound care coordinators consider and strive to address every barrier that the child and their family may be experiencing. This comprehensive approach may include case management, crisis outreach, housing assistance, counseling, medical care, education support, and more.

Family Voice and Choice
Family members' perspectives and preferences are prioritized throughout the wraparound care plan, ensuring that their input and needs guide the support provided.

Every child's individual needs and strengths are considered and validated within the wraparound plan, emphasizing positive psychological frameworks, improving interpersonal connections, and nurturing self-esteem.
