This model caters to individuals facing challenges with daily functioning, targeting disorders like schizophrenia, bipolar disorder, or those struggling with traditional mental health care methods, transportation, or treatment insight. The program focuses on addressing a range of needs, from symptom management and housing to employment and family support, ensuring holistic care for clients.
